Trying to gather documentation for CRSC

Recently medically retired with 70% DOD and 100 % VA disability. I have VA ratings documentation, DD214, Orders but am unclear of what else i need to send in. Aside from CIB, letters from peers documenting combat incident resulting in injury, some medical reports and recommendations for pending Purple Heart from superiors, I don't know what else to submit. Don't know how to get DA 199 and don't know what they mean by the MEB/PEB documentation. Suggestions welcome. Thank you.

Not everyone who applies for CRSC will have ALL of the documents shown below.

The Army recommends.

Send to CRSC board:
1. copies of DD 214/215s, as well as All VA-rating decisions including the VA letter,
2. the VA rating decisions and the VA code sheets, if available
3. Medical Records from MTF or VA physicians only, unless referred to a private physician by the VA,
4. other typical official documents include,
--service medical record extracts,
--military personnel file extracts,
--military personnel data system printouts,
--prior military disability board decisions,
--line of duty determinations, safety mishap (accident) reports, next of kin notification (Western Union Telegrams),
--casualty reports, morning reports, duty status reports, order/travel voucher, official documents not in the military personnel record, etc.
--Any PEB/MEB material (documents) that addresses your disabilities. DA Form 199 (Physical Evaluation Board Proceedings. Note: Not everyone will have a DA Form 199 (Physical Evaluation Board Proceedings. Many retirees who receive CRSC did not go through a PEB/PEB. I didn't. Consequently, I do not know how one receives a copy of DD 199; perhaps from the PEBLO?

5. Medical records or documentation should show "HOW" the injury occurred.

6. Obtain a claim form DD Form 2860, Claim for Combat Related Special Compensation (CRSC) Directives Division Select DD Forms and choose 2860.

7. Submit claim, along with all supporting documentation and medical records to the service concerned.
