ICD9: -Lumbago with sciatica
-Sacroillitis not elsewhere classified
*Failed back surgeries x2 and all other conservative therapies, medications, and procedures were unsuccessful. Rough estimate below:
1. 6 Epidural Steroid Injections
2. 5 Sacroiliac joint Steroid Injections
3. 4 denervations
4. Single level lumbar decompression and fusion
5. Left SI joint fusion
6. Restricted to 4 hour work days (2 years total)
7. Aprox. 232 days of SIQ/Con leave completed in 2 years due to physical ailments

Trials for the spinal cord stimulator start on 23May 2016. If successful, surgery for the implant will be forthcoming at a date TBD.
Any insight and advice would be greatly appreciated for those who are familiar with such a situation regarding spinal cord stimulators. I have been advised that if the trials are successful, I will be deemed non-deployable (due to the nature of the implant) once the permanent implant is placed.
There is new technology recently approved by the FDA called NEVRO, a HF stimulator. Treating physician from Naval Hospital strongly advised that my condition warrants prime benefits from the new technology. Navy is currently reviewing the product.
