Please review my NARSUM

Can anyone give me any advise, or ideas about the current NARSUM I posted last week.
I will see my Doc in a couple of weeks and she will submit after we meet. She's gonna wait for next blood results, etc. So, I can ask her to restate a few things differently. I have already found some minor stuff that needs correction.

Let me know how this compares to whatever you've seen and what your opinion is on the outcome. Much appreciated.

1. Diabetes mellitus x2, uncomplicated and controlled with initiation of Byetta recently to optimize control and for weight reduction. Prognosis is good with continued compliance and blood sugar monitoring and diet modification. Exercise, unfortunately, is decreased due to right knee issues.

2. Hyperlipidemia, mixed. Patient has met with dietician for improved diet control. Patient also started fish oil 2 grams a day, and lipids will be evaluated prior to next appointment. Prognosis is also good with continued modification diet.


3. Transaminitis elevation, no known prior history. Workup underway. Patient started vitamin E 800 IU a day. Most likely etiology is nonalcoholic steatohepatitus, but workup is currently being done.

4. Patellofemoral syndrome of right knee, status post debridement of patella with chronic pain. Patient is to be evaluated by Orthpedics, and an addendum to this MEB done through Ortho.

5. Sleep Apnea, obstructive, on BiPAP. Prognosis is good with controlled BiPAP use.
