Please help..."potential disqualifying medical condition..." 16 years service

Hello,

I am currently Air Force Reserves with about 4,500 points. I have 11 years Active Duty Air Force and 5 good reserve years.

I received noticed that I maybe disqualified due to a medical condition and don't know how to respond to whether I should elect to go through a PEB or not. I am 100% Rated by the VA and was hoping to finish 20 years in the Reserves to get a reserve retirement. I wouldn't mind being medically retired but not sure if that is an option for me at this point due to the type of letter I received. Also, if I was able to meet a PEB, could that affect my VA rating?

At this point, I would love to get input on what and how I should respond.

Here are the details from my letter:

" Your medical record is being reviewed for a potential disqualifying medical condition. Please review the documents included with this letter, sign/initial where applicable and return to this office within 60 days of this memorandum....

Potentially unfitting diagnosis that requires evaluation for continued military service IAW 48-123.
1. Patellofemoral Chondromalacia Left Knee
2. Paralysis of upper radicular nerve group
3. Spondylosis and mild back pain"

I've been asked to sign a Physical Evaluation Board PEB election form which states...
1. I understand that I may request my case be forwarded to the PEB for review in the event that I am medically disqualified for worldwide duty by HQ AFRC/SGP. The review by the PEB is only to determine my "fitness" for continued military duty and not to determine if I am entitled to disability processing. By completing and signing their form, I am making my selection for PEB review.

2. If I select to have my case reviewed by the PEB solely for a "fitness" determination, I understand referral of my case does not constitute a disability evaluation, and an "unfit" decision does not entitle me to disability compensation from the Air Force.

3. I understand that I have a right to rebuttal on any "unfit" decisions made by the Informal and Formal PEBs. The Secretary of the Air Force Council (SAFPC) makes the final decision when a member rebuts an "unfit" decision from the FPEB. All decisions made by the SAFPC are final and I have no right to rebuttal. I understand that a "fit" determination may not be rebutted.

4. I further understand that if I am medically disqualified for worldwide duty by HQ AFRC/SGP and am found "fit" by the PEB and returned to duty by direction of the Secretary of the Air Force my continued participation in the Air Force Reserve will be in a restricted status. HQ AFRC/SGP will place restrictions on my participation which are appropriate for the tip of medical conditions I have.

5. My PEB selection is indicated below:
Yes, I desire to have my non-duty related, medical disqualification case referred to the IPEB solely for a fitness determination. I understand I can submit a statement for consideration by the PEB.

No, I do not desire to have my case referred to the IPEB. I understand, in the event that HQ AFRC/SGP determines i am medically disqualified for world wide dat I will be processed for involuntary separation without my case being reviewed by the PEB. If eligible for retirement, I understand I will be afforded the opportunity to apply for retirement in lieu of separation.

Any help or direction would be greatly appreciated. I am uncertain which is best to respond...yes, or no to #5.
