Opinion on DA 7652 (Commanders Statement)

I was just forwarded a basically blank DA7652 and asked to fill in the blanks as best as possible. Should I be commenting on functional limitations or should I leave that up to the CBWTU commander?

I have had virtually zero interaction with the CBWTU Commander (would not know me from Adam) nor have I ever had any interaction with the Reserve unit I was attached to for mobilization purposes.

Any advice?
 
You do not fill this form out yourself. It's asking for someone in charge to give a statement on your limitations of duty. The correct task to do is route the form up the CBWTU Chain of Command.
 
I sat down and had a meeting with the person who was filling mine out. This is a critical document in this process (or so I have been told), and the wording of it is extremely important (again, this is just what I have been told and may be different for your branch).
They might just be asking for your input on the questions, as there are some on there that the commander may not be able to answer, such as the question about whether the member wishes to remain on active duty, how many hours of work you miss per week, etc.
I was happy to be given the opportunity to give my own inputs for this document, because my commanding officer has not only never met me, but likely has no idea what my actual job requires me to do, thus would not be able to give an accurate assessment of my ability to perform.
I would just ask the person who sent it to you to clarify what action you are supposed to take.
 
I would definately provide an input for this. Of course, the cdr is at complete liberty to make it read any way he/she would like. I would guess that they would be grateful for some kind of idea on how your condition affects your work.
 
Complete it how you want it to read, then get with your supervisor...he/she should be completing this for you but giving them a completed document not only shows initiative (and the fact that you take your situation seriously), but could prevent it from being buried and not completed (like an OPR/EPR/performance review). I'm still in the process myself (waiting on fit/unfit and VA appointments), but what I have learned from others on this forum is that you have to be proactive...it's your career/life.

I completed mine myself. I had my Commander bless off on it. The only one that really knows the impact of your condition/s is you.
 
Fill it out yourself, have them review it and make changes as they see fit. Only you know your limitations. They are supposed to do it but as we all know, most won't know you from Adam.

Not true. I wrote mine as well and my name is Adam. LoL

Hey Brian they sent it to him..... I guess in your world everything is dress right dress ..... Our world is not so clean.... they gave it to him because they want his input... when he's done HE WILL SUBMIT IT BACK TO THE CBWTU C of C (Where It Came From).

If I was a Veterans Advocate at a base that's always under investigation I would think that your plate would be full cleaning up your own backyard. ;)
 
I wrote mine myself just like others here have done in the past. In fact the only reason I did write mine myself was after joining this forum and receiving this same advice. It worked our great and all my CO did was make a few minor changes. I know this had a huge impact on my being fond unfit. If I was attempting to remain in service I would have been able to do the same and write the commanders statement expressing how I should remain on active duty. If it wasn't for being denied recass during my MMRB, I would have requested to remain on active duty and wrote the CDRs Statement to reflect those wishes. In the end I was found unfit. I would suggest though that if you are going to at least provide a draft of this document for your commander, you specifically annotate how each of your unfit conditions have an affect on your duties and obligations. The better picture the PEB receives of your unfitting condition and how it actually affects your duties will have a factor on their decision.
