Need some important advice

So i have already posted threads on the status of my MEB and one of the advice given was to give my VA rep on post a copy of my kids birth certificate and SSN cards for VA exit. The problem I'm having is that me and the Ex are not on speaking terms and not answering my emails/ phone calls and also is living on the other side of the country! My OMPF is jacked up and only have one of my kid's birth certificate on there, so is there anyone that can give me sound advice on what I can do? My ratings are done and I'm just waiting on the 199 to be sent down, so basically my time is running short. Thank You
 
Get in touch with DEERS. Were they added into DEERS? You can obtain the SSNs from the 1172 printouts. If not, contact the hospitals or counties where they were born and request them. If you are on the birth certificate there should not be any issues in the request. You can also request copies of the cards through Social Security if they are minor children. As long as you are following all court orders there should be no issues in obtaining the documents even if you do not have joint custody.
 
The form you will need to fill out is the 21-686c

It is a declaration of status of dependents. You will need to fill one out for compensation and pension, as well as VA education benefits and VA medical benefits (if so desired).

The C&P one will be given to your MSC (make sure he inputs it).

All you will need is the name of your child, place of birth of your child and SSN. There is no requirement to provide copies of the social security card and birth certificate.

In my opinion the biggest issue that I have seen is that the MSC is not inputting the 21-686c, thus when claims are being finalized they are being paid at the single rate.
