Below is the text from the DA 7652. I took it to legal and they said it was good. If you see a glaring problem let me know as my exam results have not been received by my PEBLO, so she could accept an updated or corrected one.
My desired end state is to be be found unfit and medically retired:

Conditions that are significant and that I am claiming (there are other conditions but these are the most important in my view):
PTSD with anxiety and depression
Rectoceles/Cystocele and residuals
Migraines
Fibromyalgia
R shoulder (over 10 procedures, loss of ROM and constant pain)

I have been CPT B's commander for 13 months. In that time I have observed her duty performance on a daily basis. She has consistently demonstrated symptoms related to her PTSD. She suffers from bouts of depression that render he incapable of performing even the most basic of work skill and often results in her leaving the office early or not reporting till later in the day. Her chronic anxiety is such that she is easily startled often creating a disruption in the workplace or creating an ineffective professional environment. Her lack of concentration results in excessive amounts of time spent on tasks. She is often distracted and many times must be prompted on her assigned task. Her depression, anxiety and PTSD symptoms render her unable to maintain a professional demeanor with subordinates, peers and seniors.
Section III D. Comment Section
-Soldier does not perform duties in MOS: SM is unable to maintain effective professional relationships.
-Soldier's medical conditions/limitations affect unit accomplishing the mission. SM can not perform basic Soldier skills or work effectively with others to perform mission.
- I do not recommend retaining this Soldier: SM can not perform basic Soldier skills or effective teamwork skills.
Item III B. 2B: She can not lift her left arm high enough to fire her weapon. SM has not qualified on her assigned weapon since 2009. Soldier has had numerous surgeries on her right shoulder and already has a permanent profile. Her ability to use her shoulder prevents her form lifting her arm to fire her weapon. She cannot lift anything more than 5 lbs with her right arm. She must often take breaks in working on her computer due to numbness and increased pain. She is in constant pain from her right shoulder and has recently developed arthritis in her neck that is another source of chronic pain. She can no longer carry her equipment on that shoulder even if her pelvic conditions allowed her to do so.
Item III B. 2A. She can not lift more than 30 lbs because of her rectocele/cystocele and the resulting limitations. She suffers from chronic pelvic pain resulting in increased lethargy and an inability to respond appropriately in the workplace. She has developed residuals from her surgery that prevent her form running at all, lift anything or wear her gear.
Item III B. 2D. She cannot maintain effective professional relationships because of depression, anxiety and PTSD symptoms. She is unable to work under pressure, make sound decisions and complete her tasks in a timely manner. Her anxiety has a negative effect on the workplace further reducing her effectiveness as a member of a team. She avoids all social interactions; limiting her ability to bond wit her peers and seniors. When she is experiencing a bout of severe depression she is unable to function on the most basic levels and remains home.
Item III B 2C. She suffers from debilitating migraines that require her to leave or delay work up to 2 times a week. When she gets them she must leave work to go home to bed and she suffers from lingering affects after the migraine has passed. SM also suffers from widespread and diffuse pain and exhaustion, she was diagnosed with Fibromyalgia in August 2013. The affects of the Fibro require her to leave work early on a daily basis.
Item III B. 3A SM's effectiveness, concentration and ability to complete tasks greatly diminish when she is stressed. For example learning new tasks, impending deadlines or additional workload. I have observed these symptoms and have had to alter her environment in an attempt to maintain her professional output with limited results.
