I have also found myself in a position where I am being informed by the state mental health provider that I just need to fill out this form. My previous commander is willing to help, but I want to make sure I hit the right areas. I am in a senior NCO position and diagnosed with PTSD (chronic sleep, anxiety) for more than 13 years now. I have finally said F*&^ it! and started the process. Any examples would greatly be appreciated.
