Help needed Mental Health

Hello everyone. I just looked through my records and saw a lot pf MH codes and things I don’t understand. How does these look to VA/PEB and claims. There’s some numbers attached to them and notes.

The info in the pictures are names and abbreviations of instruments (test) for different issues/disorders. I recommend you talk to your MH provider and ask him/her to expand the results of each of these instruments. Do your records reflect any diagnosis/diagnoses?

I went through I see diagnosis
Suicidal Ideation
Adjustment disorder
Anxiety mixed with Depressive mood.
Btw I was hospitalize and diagnosed with the SI. I have off post psychiatrist as well but still waiting for records as well.
