HELP! DA 7652 Commanders Functional Statement HELP!!!!

Scocamhunt,
Jason gives you a good example above. I concur with his advice.

Redstripe101,
No one said the CO is going to sign it without working on it some more. This soldier has a chance to lay out the facts from his point of view. Its a golden opportunity. If he follows Jason's advice above he will have a great CO's letter, which weights heavily in the soldier's favor. Furthermore, slamming his CO doesn't answer the brother's question.

my question is where on this form do you put anything other than mental health? section III B is for mental conditions and Section III C is for TBI. I dont see any other sections to input physical problems... this form makes no sense.

If I recall accurately, my immediate supervisor used "Section III D Comment Section" for additional input referencing my physical disabilities affecting duty performance.

Moreover, I suggest that you make an inquiry to your PEBLO and/or the profiling MEB physician for additional assistance. Take care and enjoy life!

Thus, I quite often comment that "possessing well-informed knowledge is truly a powerful equalizer."

Best Wishes!

Writing my own 7652 as well. Any "examples" out there anyone is willing to share? I think I've got the general gist, but think seeing a "complete" 7652 would give a good starting point. Thanks in advance.
