DA Form 7652 help required

So I am just starting up my MEB process and my commander has sent me the DA Form 7652. He asked me to fill it out as much as I am able to, he will review and sign it. I am going through the MEB for my hip.
If I am not mistaken, the form is only to speak on how my duty limiting injury is affecting my work performance. The form does not care about stuff that I plan to claim, but instead is only focused on what issue got me here in the first place.

If anyone has an example that they could send, I would be forever in your debt. Thank you all!

That is awesome! Yeah only speak about unfitting conditions that affect your job. However, this is your chance to help your case. If there is any other condition that prevents you or limits you being able to do your job make sure its listed and how it affects you. This will help you fight to have the condition added as unfitting later down the road in the process.

It's a win/win when commanders allow you to write it up. You know more about your situation than they do and all they have to do is read it and if they agree with whats written they will sign it.
