Does anyone have a sample reconsideration appeal letter as an example? The one under download the link is broken! I have until Wednesday morning and the sample the PEBLO gave me was crap. Please help.

What do you want the MEB to reconsider? Found fit and Return to Duty? Or Medical conditions not evaluated? Either way the biggie is to provide medical notes as evidence.
Formal Request for Reconsideration of MEB finding of ---------
I formally request reconsideration of the Medical Evaluation Board finding of---------.
I submit the following evidence for consideration of medical conditions which contradict a finding of ---------.
I also submit a Letter of Exception as to the accuracy of Dr's ----- notes of (date).
(Letter of Exception state what the doctor's notes states and what you disagree with as to what occurred, Dr did not perform evaluation as stated in notes, The Dr did not state this or stated to me something contrary to his notes, I did/ did not agree with Dr at the time, Dr omitted my complaint of ----------, If the Dr pulled the Waddell Test on the first exam or pushed on your head instead of your shoulders and you complained of back pain, ,. Especially if Dr performed the range of motion exercises and asked you to state where to stop. Basically where the SOB's lied.)

Thank you for the advise. I got my letter done and the PEBLO said it was the best she ever seen.
