The DOD is rated 40% w/bilateral factor but I assume you mean VA. Doesn't the VA have to take the most current and relevant test (as in performed by a more competent doctor)?

You want as much documentation as possible for the VA. Claim EVERYTHING. Reason being is that the Air Force will only rate you for UNFITTING CONDITIONS (IE the reason(s) you were referred to the IPEB anyways). So you may have 15-20 items like me with the VA but only got evaluated by the Air...
The older ID cards say INDEF but the newer retiree cards will have an expiration. We were told not to confinscate IDs from retirees because they rolled back the INDEFs in DBIDS for entering military installations (AF side).
I believe it depends on the character of discharge. If you just get NJP'd I still believe you will be extended to finish your MEB. But if its a DD/BCD then I believe you pretty much get let go. Not 100% on the BCD part.
